RENEWALS: A license cannot be renewed until it is WITHIN 30 days of expiration. PLEASE keep this in mind when scheduling your appointment. There is a $25 reissue fee.
- PLEASE download and review the: "Patient Telemed Instructions" below under the "DOWNLOADS" Section. Following this guide will make your appointment go smoothly and efficiently.
NOTICE: Your recommendation will be valid for 30 days from the day of issue. If your form expires and you need another, there will be a $25 fee to cover administrative costs to reissue the Physician Recommendation Form.
Finally: Getting a Physician Recommendation from the doctor IS NOT a Guarantee that the state will approve the license. WE ARE NOT RESPONSIBLE FOR APPLICATION DENIALS THAT ARE THE RESULT OF ANYTHING OTHER THAN THE PHYSICIAN RECOMMENDATION FORM. If there is a problem with the Physician Recommendation Form, we will remedy it ASAP at no charge.
